Tyrothricin is a topical antibiotic, that has an exclusive(!) local action and  is not systemically absorbed , therefore it doesn’t cause any toxic effects, it doesn’t irritate the mucous linings and doesn’t influence gut bacteria, also acting against a broad spectrum of bacteria and fungi; chlorhexidine is the best known antiseptic used, which has a broad spectrum of activity, that enhances the tyrothricin effect and enables the reduction of inflammation; lidocaine is the most commonly used local anesthetic for pain relief that enables its rapid fading.

To ensure an effective and long lasting action on the microorganisms that caused inflammation, Trachisan tablets should necessarily be sucked! In order to prevent removal of the medicine from the lining of the throat, it is recommended to avoid  liquid and food intake for 30 minutes after the dissolution of the tablet. Pharmaceutical form: 20 sucking tablets, 10 tablets per blister, 2 blisters in the package.

Tyrothricin : 0,5 mg;
Chlorhexidine gluconate : 1,0 mg;
Lidocaine hydrochloride : 1,0 mg.

Excipients : Sorbitol , magnesium stearate , peppermint oil .
1 tablet contains 0,609 g of sorbitol (sugar substitute), the equivalent of  0,057 bread units.

Pharmacotherapeutic group: medicinal products for diseases of the oropharyngeal region.

Indications: Inflammatory and infectious diseases of the oral cavity and pharynx, such as stomatitis, gingivitis, paradontitis, glossitis, tonsillitis, pharyngitis and other diseases accompanied by difficulty to swallow. For the prevention of infections prior to surgery in the oral cavity and pharynx (tooth extraction, surgical treatment of gums, tonsillectomy).

Contraindications: Hypersensitivity to any component of the medicinal product. Descuamative and erosive changes of the oral cavity lining; recent, extensive lesions of the  oral and pharyngeal cavity. In case of fructose intolerance, the medicinal product can be administered only after consultating your doctor.

Side effects: Trachisan is usually well tolerated. Short - term taste disorders and numbness of the tongue are possible. When used for a longer period, in unique cases, an insignificant coloring in yellow or light brown may appear on the teeth, dental fillings, dentures or tongue, in such cases it is necessary to comply with rigorous oral hygiene. The coloration disappears by itself when use of the medicine is discontinued.

Interactions: were not observed.

Overdose: there are no reported cases of overdosage.

Storage precautions: store at a temperature below 25°С, in a dry place, out of reach of children.

Shelf life: 5 years.

This medicinal product contains a combination of substances with antibacterial and local analgesic action.

Tyrothricin is a combination of several linear(aliphatic) and cyclic polypeptides with antibacterial effect. The combination contains up to 70-80% tyrocidine - an alkaline cyclic decapeptide and  20-30% of gramicidin - a neutral linear pentadecapeptide.

Tyrocidine causes the release of nitrogen and phosphorus containing substances from within the bacterial cells, substances which, similarly to the cationic detergents, destroy the osmotic barrier of bacterial cell membranes . Tyrocidine has a germicide effect on microorganisms undergoing growth and division.

Gramicidin leads to the formation of ion conducting channels in the  membrane of the bacterial cell, leading to changes in the intracellular concentration of cations and cytolysis.

The gramicidin component subsequently causes uncoupling of oxidative phosphorylation from the respiratory chain.

The action spectrum of tyrothricin covers gram-positive cocci and bacteria and various types of fungi (such as Candida albicans). Unlike antibiotics, after the administration of tyrothricin no cross-resistance of the microorganisms is observed. This is due to the particular mechanism of action of tyrothricin.

Chlorhexidine and its salts have a broad antibacterial effect on gram-positive and gram-negative bacteria.The mechanism of  chlorhexidine effect is based on its affinity to the cell walls of microorganisms, their surface properties being altered upon contact with this active substance.

The lipophilic groups of chlorhexidine lead to the disintegration of the cell wall lipoprotein membrane, disturbing the osmotic balance of cells,  with degradation of the  cytoplasmic membrane of the pathogenic cell.

The effect against gram-negative bacteria (Pseudomonas, Proteus) fungi, dermatophytes and mycobacteria is insignificant.

Chlorhexidine is not effective against fungal spores, saprogenic fungi and viruses.

Lidocaine hydrochloride is an amide-type local anesthetic. This substance blocks the sodium channels of nerve fibers, the nerve so that the membrane can not be depolarized by the action potential . This way the transmission of impulses in the conducting nerve fibers is blocked.

One sucking tablet is administered every 2 hours, allowing it to dissolve slowly in the mouth. The daily dose for adults and children over 12 years is 8 tablets, for children aged between 4 and 12 years - six sucking tablets.

Generally, the symptoms of the disease  regress after a few days, but in order to ensure a stable therapeutic effect, treatment should be administered for 2 to 3 more days after symptom resolution. If no improvement is achieved after 5 days of use, it is necessary to consult a doctor.

Ongoing treatment with Trachisan sucking tablets should not exceed the duration of two weeks.

Children – it is not recommended to use this medicinal product in children, given the fact that is must be sucked.

Pregnancy and lactation: no data is available on the use of this medicinal product during pregnancy and lactation.  

Trachisan does not contain sugar, therefore it does not cause dental caries and can be used in patients with diabetes mellitus, using the equivalent of the sugar substitute sorbitol in bread units (0,69 g of sorbitol are equivalent to 0,057 bread units).
